Job Description:
A company is looking for a Patient Enrollment Specialist to
support families affected by dementia through education and
guidance.
Key Responsibilities
Conduct proactive outreach to potential clients through high-volume
outbound calls
Perform initial phone screenings to assess family needs and program
fit
Educate families on the dementia care management program and
document client interactions in the CRM system
Required Qualifications
College degree, preferably in social work or psychology
Comfortable with technology and virtual work
Fluent in English and Spanish
Experience in building rapport with patients and families
Able to manage difficult interactions with empathy and
professionalism
